Abstract
Auther observed statistically 302 cases of hemolytic streptococcal infections in Korean children who had admitted to Dept. of Ped., St. Mary¢¥s Hospital from Jan. 1966 to June 1972.
The results were as follows:
1) The incidence of streptococcal infections in 4,739 hospitalized sick children was 6.3%. Among 302 cases, 176 cases(59.2%) were female. The annual incidence was highest in 1966(8.9%).
2) In these streptococcal infections, the incidence of acute glomerulonephritis, rheumatic fever and streptococcal sore throat were 200 cases(66.2%), 35 cases(11.6%), 12 cases(4.0%), respectively.
3) The average ages of the onset of acute glomerulonephritis, rheumatic fever and streptococcal pharyngitis and scarlet fever were at the age of 7 to 9 years, 7 to 12 years, under the age of 6, and at the age of 4 to 6 years respectively.
4) Seasonal incidence of the streptococcal infections was more prominent from September to February through a year.
5) Most common preceding infection in acute glomerulonephritis, rheumatic fever was upper respiratory infections, and the incidences were 56% and 27.2% respectively. The initial symptoms of acute glomerulonephritis developed most frequently at second week(51.3%), whereas the initial symptoms of rheumatic fever developed most frequently over 4 weeks (50%) after preceding infections.
6) Laboratory findings:
In acute glomerulonephritis, elevated E.S.R in 64.5% of cases, leucocytosis in 43.6% of cases, ASO titer over 333 units in 46.5% of cases and positive CRP in 15.5% of cases were found.
In rheumatic fever, elevated E.S.R in 61.8% of cases, leucocytosis in 43.6% of cases, ASO titer over 333 units in 63.7% of cases and positive CRP in 48.2% of cases were found.
On scarlet fever, elevated E.S.R in 83.4% of cases, leucocytosis in 100% of cases, ASO titer under 250 units in 100% of cases and positive CRP in 35% of cases were found.
7) The frequency of positive culture of beta-hemolytic streptococcus by throat aweb in acute glomerulonephritis, rheumatic fever and scarlet fever were obtained in 20 cases(33.9%) out of 59 cases, in 4 cases(28.6%) outof 14 cases, in 5 cases(41.7%) out of 12 cases, in 4 cases(80%) out of 5cases, respectively.
